The preventive consultation is an opportunity for you, as an employee, to discuss (severe) health issues and your work with a (medical assistant) occupational physician. Together, you will explore which tools and support are needed to prevent absenteeism.

Access to preventive consultations

You can request a preventive consultation independently without the intervention or involvement of your employer. Access to the preventive consultation is your legal right. Zorg van de Zaak processes the request without sharing your details with your employer. In other words, we do not inform the employer about the appointment, and the reference on the invoice is anonymous.

The (medical assistant) occupational physician only contacts your employer with your explicit permission. Not to discuss health complaints but rather to seek a suitable solution together.

Information for the employer or manager

Only the employee can request a preventive consultation. As an employer, however, you can inform the employee of its availability and suggest making an appointment. The preventive consultation is a voluntary service. It is therefore at the employee’s discretion to request the appointment. A professional will prepare feedback in some cases. This is done at the employee’s request. An employee is also free to decide whether this feedback is shared with the employer.

How do you request a preventive consultation?

Simply complete the requested information on the form and click ‘submit’. You will then receive confirmation via email and we will get in touch with you as soon as possible.
